- For ALS/PEPT
- ALS - Accreditation and Equivalency of Test Passer Certificate
- PEPT - Certificate of Advancing to the Next Level
- STEP 2
- Certificate of Good Moral Character from the last school attended
- STEP 3 (Pick One)
- Latest Income Tax Return (ITR)
- Certificate of Tax Exemption from the Beureau of Internal Revenue (BIR)
- Certificate of Indigency from Barangay
- STEP 4

- Attached all the scanned documents together with the filled up application form and sent it via email
